An independent body is to investigate how a breast cancer patient was mistakenly given the all-clear twice from tests analysed at University College Hospital Galway.

The Health Information and Quality Authority is to investigate pathology services at UCHG after the woman’s treatment was held up for 18 months because of the mix-up.

The H-S-E is also reviewing tests which were analysed at the lab during the periods in question.

The hospital has since apologised to the woman.
