The death of a Renmore-based soldier in the Lebanon over 20 years ago "could have and should have been avoided".

Ballinrobe native Corporal Fintan Heneghan and two Sligo soldiers were killed when their truck drove over a landmine while on peacekeeping duty on March 21st 1989.

The report found the operational procedures were not enough to ensure the safety of soldiers in that area of Lebanon.

It also says there was a failure to carry out an adequate threat assessment and the device which killed the three soldiers should have and could have been detected before it detonated.

Justice Minister Alan Shatter has apologised to their families.
