Galway Bay fm newsroom – A review of governance structures at University Hospital Galway is underway following the maternal death of Savita Halappanavar in October 2012.

A report on progress in implementing recommendations arising from a number of inquiries has been released by the HSE to The Irish Times under freedom of information legislation.

31 year old Savita died on October 28th 2012, seven days after her admission to a gynaecology ward at UHG.

Three reports have been carried out to examine the incident.

One of these, a HIQA report, was published last October and found there was a fundamental failure to provide basic care at University Hospital Galway.

It also stated the clinical governance arrangements within the hospital failed to recognise that vital hospital policies were not in use, nor were arrangements in place to ensure the provision of basic care on St. Monica’s ward.

It makde 34 recommendations for maternity services both in Galway and nationwide.

The progress report seen by the Irish Times says a review of governance structures took place on December 19th, 2013, and a follow-up meeting was scheduled for February 5th.
