HIQA to publish report into death of Savita Halappanavar by Autumn
Galway Bay fm newsroom – The Health Information and Quality Authority is due to publish its report into the death of Savita Halappanavar by early Autumn.
Dr Pat Nash, Clinical Director of the Galway Roscommon Hospital Group told last evening’s public meeting of the board that although no definite date had been set, it was anticipated to be late summer or early autumn.
31 year old Savita Halappanavar died at University Hospital Galway on the 28th October last year after suffering a miscarriage, 17 weeks into her pregnancy.
A report from the HSE inquiry team was published last week and stated that there were failures in the standard of care provided at UHG.
Addressing members last evening, Dr. Nash said the report was part of a safety investigation to look at causal factors.
He added that the hospital has since undertaken significant improvements including the early warning system and improvement in communication.
A vote of apology and condolence to Savita’s husband Praveen and family was passed by the board.
