HIQA to publish report into death of Savita Halappanavar at UHG

Galway Bay fm newsroom – The Health Information and Quality Authority is to publish the findings of its investigation into the death of Savita Halappanavar at UHG last year.

The 31 year old, who was was 17 weeks pregnant, died on October 28th last – a week after being admitted to University Hospital Galway.

The investigation team was tasked to examine the safety, quality and standard of services provided by the HSE to patients, including pregnant women, at risk of clinical deterioration.

HIQA is to publish its report into the maternal death at 3 this afternoon.

It’s to hold a special briefing at its headquarters in Smithfield in Dublin.

It follows a clinical review into the case published by the HSE in June.

It found there was inadequate assessment and monitoring, a failure to offer all options including a termination, and a non adherence to clincial guidelines on sepsis.

In April, the jury at the inquest returned a verdict of medical misadventure and strongly endorsed nine medical recommendations aimed at preventing similar fatalities in the future.
