HIQA identifies failure to provide basic care in Savita case

Galway Bay fm newsroom – A HIQA report into the death of Savita Halappanavar has found there was a fundamental failure to provide basic care at University Hospital Galway.

The 31 year old died on October 28th last, seven days after her admission to a gynaecology ward.

The authority identified that there was a general lack of provision of basic fundamental care, for example not following up on blood tests.

There was also failure to recognise Savita was at risk of clinical deterioration, and a failure to act or escalate concerns to an appropriately qualified clinician.

In addition, the clinical governance arrangements within the hospital failed to recognise that vital hospital policies were not in use, nor were arrangements in place to ensure the provision of basic care on St. Monica’s ward.

It makes 34 recommendations for maternity services both in Galway and nationwide.
